ClearColorFor refused every integer format, so a fast-cleared R32_UINT/R16_UINT/R8_UINT surface was uploaded through the CPU path: a guest read that waits for the GPU writers, then FillDccClear's zero texels. Code 0000 is zero bits in any format, so it maps to a zero integer clear. PPSA28416: ~1.4 s per 10 s of CPU uploads gone, 4.9 -> 5.3 fps.
The depth image is created with the host format, so the sample locations
check for the image and for the pipeline asks about that format too.

A 16-bit depth target with stencil maps to VK_FORMAT_D16_UNORM_S8_UINT,
which NVIDIA devices do not support as an attachment, so creating it threw.
The surface, its views and the pipelines now use the first of D16_UNORM_S8_UINT,
D24_UNORM_S8_UINT and D32_SFLOAT_S8_UINT the device can attach and sample.
Guest-side choices (16-bit sampled views, bias units) still follow the guest
format. A constant depth bias is scaled by 256 for D24 storage; on D32 float
storage it throws, as its unit depends on the depth value.

APS5_TRACE_FRAMES=<start>:<count> (or APS5_TRACE_SECONDS=<start>:<length>)
records every PerformanceTimer scope and stage of the queue workers and the
presenter inside the window as complete events, named after their frame
timing keys, into a preallocated buffer per thread. Slices under 20 us are
counted per thread instead of written; a full buffer stops that thread and
sets trace_dropped. When the window closes, a ProfileOutput writer thread
writes aps5-trace-<commit>-<pid>.json.gz and appends one line to
aps5-summary.jsonl. Without either switch a PerformanceTimer costs one more
predictable branch.

The memo stamp held TrackerGeneration, which every collect and driver store
advances, so it almost never matched. Driver stores into uncovered ranges
record their generation per 64 KiB block, and NewestStamp of the surface
replaces TrackerGeneration in the stamp.
Read the device-local heap's VK_EXT_memory_budget figures before a texture
or storage image allocation and pick a host memory type when the
allocation does not fit, rather than allocating again after the driver
refused the first attempt. The allocation itself is made once and still
throws when it fails.
RetiredImport now owns the dropped import's map node (std::map::extract), so releaseParkedImports
takes the parked import's range from node.mapped().

#4152 dropped the device-wide module list that #3173 added, so Pipeline
destroys the shader modules right after the library parts are created, also
for the parts prebuilt at registration. AMD's Windows driver still reads
them during the background link-time-optimized link and faults inside
amdvlk64.dll. Hand the modules of each new part to the device's library
store again and destroy them in ClearPipelineLibraries.

With PA_SC_AA_CONFIG at one sample and MSAA_ENABLE off, the hardware scans
each pixel once and gives every sample of a multisampled color target its
result: radeonsi programs exactly this when GL multisampling is disabled on
a multisampled framebuffer (si_emit_msaa_config leaves sc_aa_config 0), and
GL defines it as single-sample rasterization with full coverage. Such a draw
without depth or alpha-to-coverage now rasterizes the target's sample count
with every sample location at the pixel center, which covers all samples
exactly when the center is covered.

refreshImports and importAllocation drop an import whose registered run changed (a title growing a
heap) while the GpuMutex hold that drops it may still hold a HostImport pointer from before: a
build's in-place region (UploadFinish flushes pending images between regions, and a flush may
reconcile the imports), an indirect argument or a vertex import. The holder went to the batch open
at the drop, or was destroyed at once with the recorder idle, and the map node the pointer names
was erased. A Submit or Sync later in the hold put the command using the old buffer or address into
a later batch, so the import could be freed before that batch ran and the GPU read an unmapped
page (VK_ERROR_DEVICE_LOST; VK_EXT_device_fault reports READ_INVALID inside the dropped import).
The dropped import's map node now moves into its holder (std::map::extract), so those pointers stay
valid, and the holder is kept by every batch the same hold opens (Recorder::KeepThroughHold), so
whatever the hold records with them completes first. Drops for pages the guest releases (the lease
waiter) keep the old lifetime, and the waiter lets go of what an ended hold kept so it does not pin
the ranges.

Goat Simulator 3 draws a triangle list whose indices are all 0xffffffff
with primitive restart off. Each fetch past a descriptor's records reads
zeros (out-of-bounds select 0 or 1), but the zero-padded vertex copies
were sized to the highest index: 160 GiB, which vkAllocateMemory refused.
When the highest index is at least 64Ki vertices past the largest
per-vertex record count, the draw now uses a copy of its indices with
every index past that count moved to it. Each attribute reads zeros
there as at the original index; only the vertex index the shader sees
changes, so indices closer to the records are left as they are.

* fix(agc): keep the driver's per-thread scratch out of thread_local teardown
With MinGW's emulated TLS, a thread_local with a destructor in a DLL is
destroyed after its storage is gone: when a std::thread ends, winpthread
runs libgcc's emutls key destructor, which frees the thread's thread_local
storage, and only then does the loader's DLL_THREAD_DETACH run the DLL's
tls_atexit list, so the destructors run on freed memory. The queue worker
touches four of them in libSceAgcDriver (the dispatch-ahead map, the
pending waits, the draw-snapshot compare vectors and the image decode
memo), and Driver::stop's join of the worker then corrupts the heap.
Under cdb's checking heap agc_driver_draw_cache_reuse stopped at the
worker's exit with heap corruption in all 127 runs, with graphics
pipeline libraries, with APS5_NO_PIPELINE_LTO=1 and with APS5_NO_GPL=1.
Without it the damage shows intermittently, as exit 0xc0000374 (2 of 143
runs with a cold NVIDIA shader cache) or as a fault in whatever allocates
next, such as NVIDIA's compiler on the optimized-link thread (#2980).
These objects now live in HostThreadLocal, fiber-local storage that owns
them, as the recorder's buffers do since e424b6b7b.

* fix(libs): keep thread_local objects with destructors out of module teardown
The teardown order behind the driver's heap corruption holds in every
module: with MinGW's emulated TLS a thread's thread_local storage is freed
before the module's DLL_THREAD_DETACH runs its thread_local destructors,
so they run on freed objects. PreciseSleepUs's timer (used, among others,
by the video-out vblank thread) then closes a stale handle; libkernel's
directory path, socket waker and UUID random device, libSceRandom's random
device and libScePosixForWebKit's random device and host lookup free or
close stale objects. They move to HostThreadLocal. The other thread_locals
in the modules are trivially destructible or only built for Linux.

* test(libs): fail when a module registers a thread_local destructor
prx_thread_local_destructors lists the symbols of every built module and,
where MinGW's thread_local destructor support is linked in, disassembles
the module for calls to __cxa_thread_atexit outside libc's registration of
guest destructors. A module that keeps a thread_local with a destructor
fails it with the function that registers one: with main's libSceRandom
it reports sceRandomGetRandomNumber, and with this branch all 122 modules
pass, libc being the only one that links the support.

* fix(agc): keep the image memo behind its pointer
The image decode memo went into HostThreadLocal as the whole array. On
Linux, where HostThreadLocal is a plain thread_local, that put 1024 memo
entries into every thread's TLS block instead of one pointer. It keeps
its unique_ptr as before, now owned by HostThreadLocal, so Linux builds
the same thread_local as main.

A device replaced at AttachWindow keeps its host imports in
HostImports::parked until it is destroyed, and each parked import keeps
its guest ranges pinned. A guest munmap, mprotect or free of such a range
spun in the pin waiter until APS5_PIN_WAIT_MS and then failed, because the
waiter only looked at the live imports. The replaced device was already
waited idle by PrepareForReplacement, so the waiter now drops the parked
imports over the range.
agc_parked_import_unmap imports a range on one device, again on a second
one, and unmaps it with a 3 s pin wait.

Retain weak identities and the full failed span so a replacement of either owner or a shorter remaining run retries the import. Keep unchanged failures cached across unrelated mapping changes without pinning their allocations. Existing upstream import lifetime, span windows and Windows fallback remain in place.
