fix(relinker): report the ModRM of 3DNow! instructions in decoded instruction info 
DecodeInstruction did not treat 0F 0F as having a ModRM byte, so a
RIP-relative 3DNow! operand was not reported to the analyses and the
--to-intel stub relocation that read HasRipRelativeDisp.
