Brandon Thomas 9744dbd585 Tests: forms' #417 tests appear once again 
Landing #417 and the PRs chained after it re-applied #417's three
tests with each squash, so forms/src/tests.rs defined them five times
and the crate's tests no longer compiled. This restores the file as it
was tested (each test once).
